About the Author
Russell Staiff holds a PhD in art history from the University of Melbourne where he was the foundation lecturer in the postgraduate visual arts and tourism program. He began his life in heritage and tourism as a tour guide in Italy. Currently, he teaches in the heritage and tourism program at the University of Western Sydney and Silpakorn University, Bangkok. He researches the various intersections between cultural heritage, communities and tourism with a particular emphasis on Southeast Asia.

In 1922 an Irish-American adventurer named Robert Flaherty made a film about Inuit life in the Arctic. Nanook of the North featured a mythical Eskimo hunter who lived in an igloo with his family in a frozen Eden. Nanook’s story captured the world’s imagination.

Thirty years later, the Canadian government forcibly relocated three dozen Inuit from the east coast of Hudson Bay to a region of the high artic that was 1,200 miles farther north. Hailing from a land rich in caribou and arctic foxes, whales and seals, pink saxifrage and heather, the Inuit’s destination was Ellesmere Island, an arid and desolate landscape of shale and ice virtually devoid of life. The most northerly landmass on the planet, Ellesmere is blanketed in darkness for four months of the year. There the exiles were left to live on their own with little government support and few provisions.

Among this group was Josephie Flaherty, the unrecognized, half-Inuit son of Robert Flaherty, who never met his father. In a narrative rich with human drama and heartbreak, Melanie McGrath uses the story of three generations of the Flaherty family—the filmmaker; his illegitimate son, Josephie; and Josephie’s daughters, Mary and Martha—to bring this extraordinary tale of mistreatment and deprivation to life.

In this riveting tale of Canadian bureaucracy and cultural arrogance, British journalist McGrath (Motel Nirvana) tells how in 1953 a handful of Inuit families were coerced from Hudson Bay's eastern shore and relocated 1,500 miles north to bitterly rocky and icy Ellesmere Island—the world's ninth-largest island. Sold as a humane attempt to provide a livelihood for the Inuit when fox pelt prices plummeted, the scheme was, in fact, callously political. Canada wanted to plant the flag—and some people—on the uninhabited and largely impenetrable island, over which Greenland, Denmark and the United States had territorial aspirations, particularly as the Cold War intensified. A compact history of northern life adds context to the story of horrific exile, which McGrath humanizes by focusing on Josephie Flaherty, the mixed-race son of an Inuit mother and of American director Robert Flaherty, who created the cinematic sensation Nanook of the North in the 1920s. McGrath's account of inhumane deprivation is based on contemporary documents and astonishing interviews with survivors, who after decades of pleading to be repatriated to their homeland finally forced public hearings in 1993 that shocked Canadians and culminated in the 1999 creation of Nunavut, the world's only self-governing territory for indigenous people. By focusing primarily on one family, she humanizes what has been called one of the worst human rights violations in Canadian history. Through meticulous research and interviews, McGrath embraces actual events and the greater question of white reliance on misconceptions concerning indigenous peoples. She opens with the 1920s filming of Nanook of the North, then pursues connections the filmmaker had to one Inuit family. By contrasting how the Inuit are perceived by movie audiences versus the treatment they received from government employees, she sets the reader up for the devastating conclusions revealed in the survivors' 1993 testimony before the Royal Commission on Aboriginal Peoples. Disappearance of first nation people: the Inuits in Canada.

This is a riveting account of the oppression, mistreatment, and near annihilation of the Inuit forced to live in the Northern Artic regions most recently in the 1950s forward. What the book makes clear is that this pattern of taking over aboriginal people is long and truly harsh. Ross Gibson, who was put in charge of the forced movement to the uninhabitable Ellesmore and other "islands" was not aware, but the history of forcing Inuit people from their homeland began in 1576 with Martin Frobisher, Captain of the Gabriel, whose men tried to lure the natives on board, then took one prisoner all the way back to England, where his gravesites still remains.
There was no informed consent; the Inuits were told they were going to a better place, where they would have free reign, land of their own, rich hunting and fishing. What they gave up were their minimal government payments, simple medical assessment, occasional handouts of food. One of the gravest dangers when the while "colonists" explored other nations is the sharing of viruses: untreated tuberculosis, measles, unattended births - no one was trained to help deliver babies who were never warm enough and who rarely had protein, which they needed to survive.
The conference meant to symbolize apology from the Canadian government, which took place in 1993, found only a handful of survivors in attendance. Some spoke angrily of their losses; others sat listening to more promises, which have never been fulfilled. I have read this type of story before and yet, this account of the Inuit exile is most heart-breaking.

Riveting and heartbreaking all at once.

This is a fascinating and heart-breaking story of how the Canadian government moved a group of Inuit out of their homeland to a place in the farthest reaches of human tolerance for cold, darkness, and starvation. It's a revealing look down the long tunnel of greed as the Canadian government move the Inuit so they can take over their lands and harvest the fur-bearing animals there. The story reminded me that in such a climate a small problem can become a catastrophic one. Example: Often when husbands are gone hunting or trapping, nursing mothers must go foraging for wood and food. If the men are gone for weeks at a time, these supplies run out and Mom and baby can die of starvation and hypothermia. The book also reminded me that we have seen this story before within our own borders. Who knew the Canadian Government was just as arrogant and greedy as that in the U.S.?

The story takes care not to beg the issues of the Inuit; evenso, the reader still finds him/herself caring about the Inuit and their mistreatment at the hands of people whose primary motivation is money. It follows a real family through four generations. Yes it's the same old story of what happens to indigenous people on every continent, every island and in even the most cold and remote corners of the world. It led me to wonder, why is this story always the same? Why do the aborigines always get the shaft? And since I am one, the answer stings. This book looks at all the players, and all the pressures that nearly wiped out the indigenious people who lived along the edge of Hudson Bay. There is an ending of hope, however. But I'm not holding my breath.

I would recommend this book if you're interested in this kind of story. It's very well done.

Masterful recounting of a story that needed to be told

The story of the Inuit of Inukjuak and their forcible removal to Ellesmere Island is one of suffering, endurance, and ultimately, vindication and triumph. It is told with compassion and conviction by Melanie McGrath. The reviewers in both Booklist and Publishers Weekly used the word “riveting” to describe this narrative. The Long Exile is a masterful recounting of a story that needed to be told. I cannot recommend this book highly enough.

About the Author

Hank A. McKinnell joined PfizerJapan in 1971 and, in 2001, was named as the twelfth chairman in Pfizer’s 156-year history. McKinnell’s illustrious career at Pfizer spans four decades and has brought him from a country manager position in pre-revolutionary Iran to leadership of the world’s largest pharmaceutical company. Along the way, he received accolades for both his leadership ability and business acumen. MEA CULPA BY A BIG PHARMA CEO

Pfizer's CEO, Dr. Hank McKinnell has written an astonishing book in which he admits that he doesn't always believe in what he's saying [11], that drugs from Canadian pharmacies are safe [69] and that high US drug prices have nothing to do with past R&D expenses [46]. He also writes that "perhaps pharmaceuticals represent too low a percentage of total healthcare spending" [45] and he calls for "price controls to be lifted" around the world [64], because "It is time for Canadians and others to pay their fair share." [65]. He also calls for a doubling of drug patent life [185] which would result in a drastic reduction of new, low-priced generic drugs.

Dr. McKinnell starts his book with the surprising confession that he doesn't always believe in what he's saying. "They listened to my logic, but I could tell they weren't convinced, and to tell you the truth, I wasn't either." [11]

He also doesn't shy away from embarrassing facts, "Branded drug prices are anywhere from 25-100 percent more expensive in the United States." [50] He even admits, "Drugs from Canadian pharmacies are as safe as drugs from pharmacies in the United States." [69]

But his impressive mea culpa doesn't stop there. He slams everyone who makes a connection between drug prices and R&D. "It's a fallacy to suggest that our industry, or any industry, prices a product to recapture the R&D budget spent in development." [46]

Finally, in an astonishing intellectual somersault, Dr. McKinnell claims that "price controls always make prices higher in the long run." [64] And since he wants to give people lower drug prices, by eliminating price controls, he writes, "Starting with pharmaceuticals, I call for price controls to be lifted in Canada and elsewhere." [64]

Dr. McKinnell ends his book with a wonderful quote by Gandhi, for those who desire change. "First they ignore you. Then they laugh at you. Then they fight you. Then you win." [193] Dr. McKinnell just doesn't realize that he has become "them."

Big Pharma's Mea Non Culpa

If you buy the book "A Call to Action" by Pfizer's CEO, Dr. Hank McKinnell, with the notion that you will learn what this pharma executive believes the industry should do to reform itself, you are in for a big surprise. Notwithstanding an Amazon.com review of the book written by Peter Rost -- Pfizer's whistleblowing, soon-to-be non-head of Pfizer's endocrine care marketing unit and "Pharma's Black Knight" -- in which McKinnell is described as making "an impressive mea culpa," this book is actually a mea non culpa!

I had reason to expect to see suggestions for changes in the way pharma operates because at the very beginning of the book (page 13), McKinnell said that "if Pfizer wanted a seat at the table where important healthcare solutions were being hammered out, we had to earn it by acting differently, partnering differently, and communicating differently." Unfortunately, the rest of the book does NOT carry through on this theme and focuses instead on how consumers, society, employers, managed care, and government need to act differently.

Before getting to the action items, McKinnell tries to answer the most important questions put to the industry by critics and the public. These questions are:

* Why are Prescription Medicines So Expensive?
* Why Does the Industry Do So Much Advertising?
* Why Do Americans Pay More Than Canadians for Drugs?

Unfortunately, McKinnell sticks to the standard pharma industry playbook except on one or two occasions where he seems to offer contrary opinions.

Why are Prescription medicines So Expensive? It turns out, according to McKinell, that drugs are not as expensive as you think (silly you!) and that branded drugs are cheaper than the alternative - the old "an ounce of prevention is worth more than a pound of cure" defense.

In essence, his argument is, drugs are not as expensive as you think, cheaper alternatives are soon available, and drug profits are good and well deserved.

I should acknowledge at least one refreshingly contrary view expressed by McKinnell: an admission that it is a "fallacy to suggest that our industry...prices a product to recapture the R&D budget spent in development." He suggests instead that it is "investors' confidence in the risk and rewards" that drives prices.

McKinnell takes exception to critics such as Senator Bill Frist who claim that "increased direct-to-consumer advertising has helped fuel escalating drug costs." McKinell, like PhRMA, insists that "studies show there's no correlation between prices or price increases and DTC advertising."

McKinnell cites a study done in 2001 by The National Institute for Health Care Management entitled "Prescription Drug Expenditures in 2000: The Upward Trend Continues." McKinnell claims that this study shows that the "average price of drugs that are advertised is lower than those that aren't." The report, however, concludes that the increase in prescription drug spending is due, in part, to "more aggressive marketing of prescription drugs to both doctors and consumers."

Why Does the Industry Do So Much Advertising? McKinnell wants us to believe that DTC advertising is really education (he prefers ro call it "DTC education"). This is not a new mantra. Johnson & Johnson Chief Executive William Weldon, for example, said in an address to PhRMA: "Our communication with patients should really be thought of as Direct to Consumer Education."

Why Do Americans Pay More Than Canadians for Drugs? To be honest, I didn't read this chapter. According to Peter Rost, "McKinnell ... admits that drugs from Canadian pharmacies are safe ...[and] that 'perhaps pharmaceuticals represent too low a percentage of total healthcare spending' and he calls for 'price controls to be lifted' around the world, because "It is time for Canadians and others to pay their fair share."

Chapter 13 is the chocolate nugget of the book. In this chapter McKinnell lists 10 actions that he believes should be taken to reform the healthcare system in the U.S. Only one action on the list, Reform Direct-to-Consumer Advertising, is an action item for the drug industry. All the others are the responsibility of individuals, society, employers, managed care, and governments, not the pharmaceutical industry. Coincidentally, at least six actions would either benefit the pharmaceutical industry in general or highlight specific Pfizer initiatives. Rather than focus on these - you can read more about it in the Summer 2005 issue of Pharma Marketing News - I'd like to focus on Mickinnell's suggestions for reforming DTC.

Reform Direct-to-Consumer Advertising
This is the one action item mentioned in the book that applies directly to the pharmaceutical industry and can be said to require a change in the way it does business. Here's what McKinnel recommends for DTC:

* Improve communications about risk
* Provide information about alternative therapies available, including therapies that do not require a medicine
* Promote prevention and wellness, compliance with treatment regimens, and disease awareness
* Promote access to medicines and do a better job communicating about patient assistance programs
* Implement external oversight of DTC, including working with the FDA to get their views on advertisements before they are run

Mckinnell's action plan for DTC reform, however, does not go as far as some other emerging principles do. He does not suggest, for example, a 1-year moratorium on DTC for news drugs or to limit TV ads to the appropriate audience at the appropriate time of day (something that Congress may impose on ED ads). Both of these principles have been adopted by Bristol-Myers Squibb in its Direct-to-Consumer Communications Code. It should be noted, however, that McKinnell has said in another venue "No erectile dysfunction drug ads on television except for 10 pm to 6 am. I'm in favor of that." (Washingtonpost.com Viewpoint interview, 2004).

Needless to say, this book is not going to shake up the pharmaceutical industry nor does it offer much new defense against critics of the industry. It often sounds like an advertorial for Pfizer's programs. A Pfizer spokesman referred a reporter to McKinnell's book as the "best source" for the company's perspective on advertising. I wouldn't recommend it, however, as the best source for an action plan to reform drug advertising in general.

A Decent Book Raising Legitimate Questions

Like it or not, to discuss about the healthcare system this book raised some legitimate questions such as sick-care vs prevention-care, free riding on the global market, malpractice law suits, third party payment system, etc.

There are some industry and Pfizer PR stuffs in the book. Anyway, you cannot expect a current CEO of a company to suggest something that would hurt his company or industry. (By the way, Peter Rost seems to be an interesting exception. When he does this, you need to do some research on history and ask why.) Still Hank McKinnell discussed seriously about the overall structural loopholes and possible reforms of the system.

Don't simply blame the pharmaceutical industry as the only "bad" boy in the game. It's true that not everything the companies do seems "right" under the bright sunshine, but who does in capitalism? That's why the system is designed to have balances and checks, while money chasing is the ultimate incentive in the market and real life.

If you know fair amount of biomedical science, you understand how little we know and how difficult it is to develop a novel and really effective and safe treatment under current knowledge. Healthcare as a whole is a very large and complex system. Because of the sensitivity the system deals with, which is the length and quality of human lives, it is already under the toughest regulations among all industries.

If you are open to new ideas and want to seriously think about the healthcare, you may find this book useful to provide some perspectives.

The Downside of the Dollar Standard

Since the breakdown of Bretton Woods in the early 1970's and the end of the gold standard, the dollar has become the international reserve currency. The 20 years prior to 1970 international reserves increased only about 55%, but since 1970, with the adoption of the dollar standard, reserves have increased over 2,000%. This is primarily a result of US current account deficits, which last year ran about $600 billion - about 3% of GDP. Asian central banks hold about $2 trillion US dollar-denominated reserve assets. This surge in international reserves has created huge imbalances and it is the subject of this book by financial analyst Richard Duncan.

The dollar standard has allowed the US to finance incredibly large deficits by printing more dollars. The dollar standard has on the upside ushered in the age of globalization that has allowed Asian economies - first Japan, then the Tigers, and now China - to devolop by exporting to the US without importing equal amounts, leaving Asian central banks with large stockpiles of dollar reserves. And what can Asian central banks do with these reserves? About the only thing they can do is invest in US corporate stocks and bonds, T-bills, and US agency debt such as Fannie Mae and Freddie Mac. (We've been enjoying low mortgage rates because the Asian central banks buy up our debt so we can take out more.) And all these investments in return allow US consumers to buy more of their exports - call it vendor financing.

According to Duncan, these current account deficits and current account surpluses have already wrought havoc with the world economy: it caused the asset and stock market bubble in Japan in the 1980s; it caused the currency crisis in Malaysia and Thailand in the 1990s (Duncan was an analyst working in Thailand at the time and correctly predicted its occurence); and it is currently fueling the real estate boom in the US.

Asia's export-led strategies require that dollar reserves are reinvested in the US; this prevents their currencies from appreciating. Indeed, if the Bank of China or the Bank of Japan were to invest in the Euro or any other currency, the bankers and politicians of those countries would quickly protest because it would drive up their currencies and make their exports less competitive.

So then with the US increasing the world's international reserves at a rate of $600 billion a year, everyone is still happy for the time being. Asian countries are growing rapidly and American consumers have endless supplies of credit - using their homes as ATMs - however, this imbalance, unsustainable and in the long run, will precipitate an economic crisis. Even correcting this imbalance, if not done prudently, could precipitate a world economic slowdown.

This book was written before the recent decision by China to stop pegging the yuan to the dollar. This was a baby step in the right direction.

Duncan's analysis of the problem is very good, his policy recommendations, however, are questionable. He suggests, for example, giving global central bank status to the International Monetary Fund. That's a nonstarter for reasons obvious to Republicans. He also advocates a global minimum wage, giving workers more money to soak up excess supply. I can already hear the critics screaming no "world government."

The main problem that needs to be addressed - and Duncan stresses it many times - is that there needs to be a regulatory mechanism in foreign-exchange markets. Central banks intervene in currency markets for their own benefit - such creating an export strategy - instead of looking for ways to smooth global business cycles. China, with its revaluation of its currency, is looking to become a responsible global player - we hope. If the powers that be do not act in concert to coordinate a soft landing of the current imbalances, we will all be heading for some frightening times.

A key element in maintaining a rounded knowledge of finance!

In this book, Richard Duncan describes the inherent instability that underlies the contemporary global financial system.

This instability has arisen due to the abandonment of the gold standard in the early 70's by US president Nixon and has recently culminated in a near collapse (foretold although not chronicled in this book)of the system. The ability to borrow is now only limited by a countries perceived ability to repay its loans and as a result the US has ammassed an enormous deficit.

Up until now the deficit has been driven by a viscious circle of China's recycling of its export (to the US) surpluses into US debt securities, in turn leading to an ever widening US deficit. Fortunately this will be counterbalanced in time to come by a decrease in competitiveness in China's exports due to structural labour shortages and a rise in minimum wages. Additionally China's stated imperative is to increase its citizens standard of living and shift from an export driven economy to one driven by internal consumer demand.

Are you brave enough to read this book

I never studied Economics at college so this book became my 101 and explained in depth why foreign countries, until recently, kept buying our US Treasury Notes even though they knew we were trillions of dollars in debt. Intuitively I suspected the current bull market was on borrowed time but this book spells out in detail how our current relative affluence is about to end and nothing can stop this train wreck from happening. This train wreck is not just the end of the latest bull market but the beginning of a severe recession and depression. Richard Duncan brings together the separate catastrophes of Peak Oil, Global Warming, the Collapsing Dollar and the Loss of its Reserve Currency Status and, the Lack of American Savings to clearly explain why the good times are about over. So be warned that this book is not a comfortable read. If like me you are wanting to absorb the message quickly, rather than try to nit pick his findings, then I would fault his over-use of charts and data tables. I started skipping them after I realized they were extra detail and not essential to the theme of the book.

Richard Duncan goes to lengths to explain how we are part of a global economy and gives a plausible reason why the only way out of the coming depression, which mainly is the result of severe trade imbalances between the US/Europe and ASEAN countries, is for these third world exporting countries to raise their living standards. This will likely mean an internationally agreed minimum wage for all forms of labor. Only by using a flat playing field can free trade occur as it is intended to.

There are ways explained in this book on how to mitigate the coming extreme pain but we will all suffer a diminished life style.
